Solution Programs will unfortunately see repeats from time to time in their patient population because of a relapse or a new event.

In order to accurately account for these patients, Orion Outcomes needs you to create a new patient profile for each new entrance into your program.

You will already have that patient's profile from their first visit. You will not be using that at all.

Treat this next entrance into the program as if this patient is a totally new patient. The problem is that you will need to distinguish this new patient profile from the old one even though the patient has the same name.

In that situation, the common practice is to add a suffix to the patient's last name. For example, "Bob Smith" would be entered as first name "Bob" but last name "Smith #2" on this second program enrollment. The original "Bob Smith" profile can be hidden by selecting "Patient is no longer involved with program" on the Program tab of that profile.

This technique should also be used for sites that synchronize Orion with telemetry systems, such as Q-Tel. In Q-Tel, you will need to also give the patient a unique last name. The same "#2" suffix will work there as well. Then when Orion imports the data, it will create a new record as desired.
